How to treat gonorrhoea

The most common STI's in the UK are Gonorrhoea and Chlamydia

They can both be treated very effectively with antibiotics.

The recommended treatment for gonorrhoea by the British Association for Sexual Health and HIV, BASHH, is an antibiotic injection.  This treatment can be obtained from a sexual health clinic or your GP.

The treatment that we offer is second line treatment for those who don’t want an injection or are unable to get to a sexual health clinic.

Sex without using a condom can increase your risk of getting a STI. Also, sex toys should be washed and covered with a new condom each time they are used.

If you have been diagnosed with gonorrhoea it’s necessary to inform any sexual partners, from the last 3 months, that they may also be at risk, so that they can be screened and have treatment if required. Also, to prevent infecting or re-infecting your partner with gonorrhoea you should both avoid any sexual contact, with or without a condom, until both of you have been treated.

We recommend that you take another STI test two weeks after your gonorrhoea treatment to make sure that you have been successfully treated. Therefore, we will send you a free test kit 14 days after you have taken the gonorrhoea treatment  to make sure that you are clear of the infection. All you have to do is message us at team@webmedpharmacy.co.uk, send us a message from your account or ring us on 0161 491 1899 and we will send you a free test kit worth £45 by Royal Mail first class post.

Please be aware that you have to message or phone us no more than 28 days after you have received your treatment pack as after that time the test will no longer be considered to be a "Test of Cure".

Please keep a record of the tracking number on the postage label of the test kit before you return it to the lab so that we, and you, can monitor its progress. If you don't keep a record of it and it gets lost in the post then we can't replace it.

Also, both men and women can get gonorrhoea in the rectum, throat, or eyes by having unprotected anal or oral sex with someone who is already infected. Furthermore, conjunctivitis may occur if infected vaginal fluid or semen comes into contact with the eyes. Usually no symptoms are experienced if the throat is infected but infection of the rectum may cause bleeding, pain, discharge or discomfort. We don't offer treatment for gonorrhoea in the  throat or eyes. If you suspect that you may have a throat or eye infection you should visit your local genitourinary medicine (GUM) clinic, also referred to as sexual health clinics, or your GP.

Ingredients

The treatment for gonorrhoea that we prescribe is two tablets of Suprax(400mg) and four tablets of Azithromycin(2G). Both are taken together as a single dose.

Suprax 200mg tablets contain a medicine called cefixime. This belongs to a group of antibiotics called ‘cephalosporins’.

Suprax is used to treat bacterial infections.These include infections of the: • Ear • Nose, sinuses (such as sinusitis) • Throat (such as tonsillitis, pharyngitis) • Chest and lungs (such as bronchitis, pneumonia) • Urinary system (such as cystitis and kidney infections).

Azithromycin belongs to a group of medicines called macrolide antibiotics. Antibiotics are used to treat infections caused by bacteria.

Azithromycin is used for the treatment of bacterial infections that are sensitive to it, such as: • chest, throat or nasal infections (such as bronchitis, pneumonia, tonsillitis, sore throat (pharyngitis) and sinusitis) • ear infections • skin and soft tissue infections, with exception of infected burn wounds e.g. - infection of the tube that carries urine from the bladder (urethra) or the neck of the womb (cervix) caused by Chlamydia trachomatis.

What dosage should I take?

The antibiotics prescribed are 2 Suprax (cefixime) 200mg tablets and 4 Azithromycin 500mg tablets, taken together as a single dose.

The tablets are best taken after food or a meal to reduce the likelihood of side-effects.

Do not take any medication for indigestion 2 hours before or after taking the antibiotics as this will reduce the effectiveness of the treatment.

What to be aware of

· We recommend that you take another STI test 14 days after the end of your treatment to make sure that the STI has been successfully treated.  In fact, if you receive treatment from us for gonorrhoea, we will send you another test kit, free of charge, to make sure that you have eliminated the infection. The "test of cure" is to make sure you are clear of gonorrhoea only.

 If you have been diagnosed with gonorrhoea it’s highly recommended to inform any sexual partners, from the last 3 months, that they may be at risk in order that they can be screened and have treatment if required.

· Please do not presume you have gonorrhoea based on your symptoms. You should be tested first to make sure you definitely have gonorrhoea before being treated. We can supply gonorrhoea test kits if you haven't already been diagnosed by an STI clinic.

What are the side effects of gonorrhoea antibiotics?

Suprax

·  Like all medicines, Suprax can cause side effects, although not everybody gets them. Tell your doctor straight away or go to the nearest hospital casualty department if you notice any of the following serious side effects – you may need urgent medical treatment:

·  You have an allergic reaction. The signs may include: a rash, joint pain, swallowing or breathing problems, swelling of your lips, face, throat or tongue 

·  Blistering or bleeding of the skin around the lips, eyes, mouth, nose and genitals. Also, flu-like symptoms and fever. This may be something called ‘Stevens-Johnson syndrome.

·  Severe blistering rash where layers of the skin may peel off to leave large areas of raw exposed skin over the body. The rash may appear especially on the palms or soles of your feet

More common side-effects are :-

• Feeling sick (nausea) or being sick (vomiting)

• Stomach pains, indigestion or wind

Azithromycin

Like all medicines, this medicine can cause side effects, although not everybody gets them.

If you have any of the following symptoms of a severe allergic reaction stop taking this medicine and tell your doctor immediately or go to the casualty department at your nearest hospital

• Sudden difficulty in breathing, speaking and swallowing

• Swelling of the lips, tongue, face and neck

• Extreme dizziness or collapse

• Severe or itchy skin rash, especially if this shows blistering and there is soreness of the eyes, mouth or genital organs. If you experience any of the following side effects contact your doctor as soon as possible

• Diarrhoea that is serious, lasts a long time or has blood in it, with stomach pain or fever. This can be a sign of a serious bowel inflammation. This is something that can rarely happen after taking antibiotics

• Yellowing of the skin or whites of the eyes caused by liver problems

• Inflammation of the pancreas, which causes severe pain in the abdomen and back

• Increased or reduced urine output, or traces of blood in your urine

• Skin rash caused by sensitivity to sunlight

• Unusual bruising or bleeding

• Irregular heart beat.

Other side effects include Very common (may affect more than 1 in 10 people)

• diarrhoea

• abdominal pain

What antibiotics treat gonorrhoea?

The recommended first line treatment for gonorrhoea by the British Association for Sexual Health and HIV, BASHH, is an injection of ceftriaxone,usually in the buttock or the thigh.

We supply second line treatment for those who don’t like injections or either can’t get to a sexual health clinic or don’t want the embarrassment.

How fast does gonorrhoea treatment work?

After having taken the single dose of the combination of the two antibiotics of Cefixime and Azithromycin it should clear up within 7 days although you may still experience some symptoms for up to another 7-14 days.

However, we recommend that you should avoid any sexual contact for at least 7 days.

We also suggest that you should get tested two weeks after taking the medication (not sooner than 14 days) to make sure the treatment has been successful.

Yes. Gonorrhoea can be treated at home by taking a combination of two antibiotics in a single dose, 6 tablets in total. It is best to take them with a large glass of water after a meal or food. This will mean that you are less likely to suffer from any side-effects.

The antibiotics are 2 Suprax (cefixime) 200mg tablets and 4 Azithromycin 500mg tablets.

How long after gonorrhoea treatment can I have sex?

You should avoid sexual contact for 7 days after taking the treatment. Don't forget both you and your partner should get treated to make sure you are both clear to avoid getting infected with gonorrhoea again.  

Can gonorrhoea be cured?

In the vast majority of cases gonorrhoea can be cured. However, please be aware that you can be re-infected if you have unprotected sex with an infected partner.

Also, you may have seen or heard in the press recently of an antibiotic resistant strain of gonorrhoea, of which there has been a few cases reported in the UK. To prevent this situation escalating it’s very important that you have been positively diagnosed with gonorrhoea before obtaining the treatment.

Many STIs have similar symptoms so it’s important to be properly diagnosed and receive appropriate treatment.  

Does doxycycline treat gonorrhea?

No. Doxycycline is an antibiotic used to treat Chlamydia over a seven day course. See the Chlamydia section for treatment for Chlamydia.

Does gonorrhoea treatment affect periods?

How do I get gonorrhoea and what does it do?

Gonorrhoea can be contracted by having unprotected sex with an infected person. If you are concerned that you have put yourself at risk then you should wait 14 days before taking a test as it may take this amount of time before the infection can be detected.

Gonorrhoea, is caused by the bacterium Neisseria gonorrhoea and is the second most common bacterial sexually transmitted infection (STI) in the UK.

The number of gonorrhoea diagnoses increased by 15% between 2012 and 2013. The infection is sometimes referred to as "The Clap". New diagnoses continued the sharp rise seen in recent years, exceeding 29,000 cases in England alone during 2013. Men and women can both be screened by using the test kits available from us and if necessary, can be supplied treatment after completing a short medical questionnaire. The treatment will be discreetly delivered the next day in a very convenient 1 hour time slot. 

 About 50% of women and 10% of men are unaware that they’re infected as they don’t experience any obvious symptoms and this means that the infection can go untreated until the disease has progressed. This can lead to serious long term health problems and infertility.

If symptoms are experienced, they usually occur about 10 days after being infected and the most common are listed below.

What are the symptoms in men?

·  Burning sensation/pain when urinating

·  Inflammation of the foreskin

·  An unusual discharge from the tip of the penis that may be green, yellow or white.

· Tenderness or pain in the testicles

·  Burning sensation/pain when urinating

·  Bleeding between periods and/or heavier periods

·  A change in vaginal discharge which may be thick and green or yellow in colour.

· Lower abdominal discomfort

· Bleeding after sex

What is the testing procedure?

If you order a STI kit from us at Webmed Pharmacy by 4pm Monday to Friday, you will receive it the next working day delivered by DPD*, for all mainland UK except the Scottish Highlands. There is no additional cost for postage unless you wish it to be delivered on a Saturday or Sunday where there is a small charge of £4.

The kits are posted in discreet plain unmarked packaging with no indication of their contents. You will then send your sample, in the pre-paid envelope; to The Doctors Laboratory (TDL) in London or Manchester. They are the largest independent providers of clinical laboratory diagnostic services in the UK. Once the lab has received your sample, your results will be available 2-3 days later.

If your test is positive for Chlamydia we will ask that you complete a simple medical questionnaire that our Doctor can assess for your suitability and, if approved, be prescribed the oral antibiotics to treat the infection.

If positive for gonorrhoea we can signpost you to your nearest sexual health clinic or supply treatment if it’s suitable and safe for you.

If you or your partner have already tested positive for a STI by using a home test kit from another site or from visiting a sexual health clinic then you may still complete our simple medical questionnaire that our Doctor can assess for your suitability and, if approved, you can be prescribed oral antibiotics to treat the infection.
